Source-Layer Position
Candour is not a single UK-wide operational rule. The current evidence supports England CQC Regulation 20 for registered providers, Welsh statutory candour provisions, Scottish organisational duty of candour, professional candour duties through GMC/NMC material, and a Northern Ireland boundary where current evidence supports consultation / Being Open direction rather than a confirmed enacted statutory equivalent in this wiki.

What It Does Not Prove
Candour evidence does not prove general record quality, provider registration, clinical safety, data protection, DSIC compliance, or supplier implementation. It only defines the incident openness, explanation, apology, follow-up, correspondence, and learning records that may be required.
Deployment Evidence Needed
For digital workflows, prove incident classification, duty-of-candour trigger handling, notification templates, apology/follow-up correspondence capture, review/action-plan workflow, audit trail, reporting, and nation-specific policy alignment.
