UK Healthcare Connectivity Landscape
This page explains how UK healthcare networks, identity, clinical exchange and portals fit together, with programme transitions and InterSystems’ documented role kept in context. For the recommended reading order, start with the UK Core Services Executive Summary, use the UK Core Services Matrix for the detailed comparison, then return here for the wider service model.
Public-source baseline: 15 September 2026. Editorial alignment: 1 October 2026. Later findings remain dated in the matrix and linked country pages; this alignment does not establish a fresh nationwide deployment review.
There is no single UK clinical backbone or exact equivalent of England's Spine in every nation. Compare the job a service performs, then check its national scope, interface and delivery status. Some infrastructure is shared: Spine explicitly serves England, Wales and the Isle of Man, and Wales uses Spine/PDS for electronic prescriptions.
The summary and matrix follow the same topic order: core services; access, registration and GP transactions; urgent-care routing; safety, prevention and care planning, including safeguarding; coding, reporting and payments; DSIC; and cross-nation reuse. The function guide below connects those comparisons to the detailed pages. Their System, Portal and other access distinctions describe specific routes, not blanket access to a national record.
The UK Clinical Platform Estate maps named shared-record, hospital EPR/portal and diagnostic products onto current regional geography. Keep that provider estate distinct from the national connectivity services described here.
Choose a Nation
| Nation | Main connectivity routes covered | Drill down |
|---|---|---|
| England | HSCN and internet access; Spine, PDS, CIS2, ODS/SDS, MESH, GP Connect, NCRS and its SCR/other features, EPS, e-RS and record locators; pathology and imaging programmes. | England Spine and National Services |
| Scotland | Scottish Wide Area Network (SWAN); CHI; Scottish Care Information (SCI) Store and Gateway; ECS/KIS; National Digital Platform and MyCare.scot; imaging infrastructure. | Scotland Connectivity Equivalents |
| Wales | Public Sector Broadband Aggregation (PSBA); Welsh Demographic Service; Welsh Clinical Portal, Welsh GP Record and communications gateway; National Data Resource; laboratory and radiology programmes; NHS Wales App. | Wales Connectivity Equivalents |
| Northern Ireland | HSC connectivity; Health and Care Number and Northern Ireland Digital Identity Service (NIDIS); NHAIS registration transition; NIECR, encompass, EpicCare Link and My Care; NIPACS+. | Northern Ireland Connectivity Equivalents |
The country pages provide the source-level service and status details. GP Connect and Spine Equivalents by Nation retains the more detailed functional comparison and four-nation adapter model.
Understand the Layers
These are different parts of a working connection. A network carries traffic; an identity service identifies a person or system; a clinical service exchanges particular information; a portal presents it to a user.
| Layer | Question it answers | Examples and tools |
|---|---|---|
| Network and secure access | How can systems reach each other? | HSCN/internet routes in England, SWAN in Scotland, PSBA in Wales. Connectivity alone does not authorise access to records. |
| Patient identity | Which patient does the information concern? | NHS number/PDS, Scottish CHI, Welsh Demographic Service, NI Health and Care Number/NIDIS. Matching and cross-border reconciliation need their own rules. |
| Staff, organisation and system identity | Who is requesting access, from which organisation and application? | CIS2 staff authentication, ODS organisation identifiers, SDS system/endpoint directory in the England route; each national service has its own access arrangements. |
| Exchange and discovery | How is information delivered or found? | MESH store-and-forward delivery; SCI Gateway and Welsh Clinical Communications Gateway; GP Connect APIs; National Record Locator; emerging National Imaging Registry. |
| Clinical record and application | Where is information held or presented? | GP/EPR/laboratory/PACS source systems; NCRS brings together SCR and other national information/functions; SCI Store, Welsh Clinical Portal, NIECR/encompass, shared care records and patient portals. A viewer is not necessarily the source of the record. |
| Content and interoperability | What do the receiving systems understand? | FHIR profiles, HL7 messages, DICOM images, IHE exchange profiles, clinical terminology and record-content standards. The named version and workflow matter. |
The layer model is this wiki's synthesis of the national-service documentation, not an assertion that all four nations use the same architecture. England's service catalogue, the three country pages and Standards and Interoperability provide the underlying evidence.
Current Services and Changes Under Way
Live means an official source describes operational use within its stated scope. Pilot/private beta means limited use. A planned date is a target, and a phased replacement does not establish retirement of every legacy function.
| Area | Documented position in the linked evidence | What that means |
|---|---|---|
| England Spine access | Spine is live. Internet access is available and NHS England encourages migration from HSCN; service-specific onboarding still applies. | Spine's access transition is not retirement of every Spine service or immediate internet availability of every interface. See England. |
| England pathology reporting | The FHIR reporting programme is in pilot; NHS England's July 2026 plan targets assurance by April 2027 and later national rollout. | Do not describe a completed national switch from legacy reporting. See pathology reporting for the current implementation guide, transport and migration boundary. |
| England radiology/image exchange | NHS England reports limited National Imaging Registry beta use in three imaging networks, with national rollout planned for 2026–27. | It is a federated discovery/access service intended to reduce reliance on IEP. Completed national IEP replacement is not established. See diagnostic exchange. |
| Scotland | SWAN, SCI Store, SCI Gateway and CHI have distinct network, repository, messaging and identity roles. MyCare.scot has initial web/mobile availability, with staged features. | A new network contract or national platform does not mean all board systems have migrated or that every patient-facing feature is live. See Scotland. |
| Wales | National portals and demographic services sit alongside phased laboratory and radiology modernisation. Welsh EPS reuses Spine/PDS. | LIMS and RISP progress must be read by site and delivery phase. See Wales. |
| NI NIDIS/NHAIS | NIDIS phase 1 identity services have been live since November 2023; replacement of NHAIS GP-registration functions is a separate phase. | “NIDIS replaces NHAIS” needs a function and date: NHAIS is not evidenced as fully retired. See Northern Ireland. |
| NI diagnostic imaging | BSO announced closure of the NIPACS+ programme and transition to operations for the end of July 2026. | Read the announcement date and delivered scope separately from confirmation of the final handover; this programme is separate from NIDIS and encompass. See Northern Ireland. |
Follow a Function
| What you need to understand | Detailed wiki coverage |
|---|---|
| Patient identity, staff login, directories and Spine connections | England national services, then the relevant country page. |
| Read a GP record, send a document or update a record | GP Connect Deep Dive, NHS Connectivity and GP Connect and four-nation comparison. These are different capabilities. |
| Manage patient sign-in, proxy permissions, registration or demographic events | Access, registration and GP transactions, with a short comparison. Identity lookup, registration changes and clinical-record access are distinct. |
| Move a GP record when a patient changes practice | GP2GP Record Transfer, including cross-nation boundaries. |
| Find or view a national/shared record | NHS National Record and Data Platforms, country services and the HealthShare component overview where supplier evidence applies. |
| Deliver a clinical message or document | MESH, GP Connect and ITK3; SCI Gateway and Welsh communications routes in the country pages. |
| Exchange pathology results, radiology reports or images | Diagnostic Results and Image Exchange. |
| Route urgent care and arrange attendance | Urgent-care comparison and its summary: access, triage, service discovery, handoff and booking are separate steps. |
| Share care plans, adjustment needs, prevention or safeguarding information | Safety, prevention and care planning, including the separate child-protection and FGM view. The summary preserves the access and national-equivalence limits. |
| Report activity, administer payments or apply data-use preferences | Coding, reporting and payments and its summary. Extraction, payment calculation and preference checking have different contracts. |
| Apply England’s GP-system requirements across nations | DSIC comparison lens and its short explanation. England assurance does not establish another nation’s interface or permissions. |
| Identify what can be reused across borders | Verified reuse relationships and their summary: shared services, platforms, administrative exchange, terminology and standards have different boundaries. |
| Understand formats, codes and implementation rules | Standards and Interoperability, Terminology Code Management and NHS Standards Directory. |
Professional and Patient Portals
A portal's name does not tell you which records it contains or what a user can change. The country sections now separate application, source information, permitted actions and availability. Patient access is a different route from a professional login; none of these portals should be assumed to expose the complete national record.
| Nation | Professional application / information route | Patient application |
|---|---|---|
| England | NCRS features and permissions, with source and launch diagrams. SCR is one source; viewing an NRL pointer, opening its document and editing a flag are different actions. | NHS App: NHS login, practice/provider services and separately authorised family/carer access. |
| Scotland | Professional and patient access: ECS/KIS summaries, SCI Store viewing and SCI Gateway exchanges retain their own sources and local access arrangements. | MyCare.scot: verified ScotAccount and an initial national feature set, with additional local rollout. See the same country section. |
| Wales | Portal features and access: Welsh Clinical Portal brings information and workflows together; Welsh GP Record and WCCG perform different jobs. | NHS Wales App: GP-enabled services and phased hospital information, with feature-specific limits. See the same country section. |
| Northern Ireland | Portal features and access: encompass and external EpicCare Link access; NIECR's transition is separately bounded. | My Care: patient access to information and supported actions in encompass, with separate proxy permissions. See the same country section. |
The linked pages carry the primary sources and precise limitations. A contextual launch opens another application with the selected patient; it does not prove that all its data have been imported into the first application. InterSystems evidence remains in the supplier-specific rows below.
Where InterSystems Is Evidenced
National-service descriptions above are independent of supplier choice. These are specific InterSystems evidence examples, with links to their scope and limitations.
| Setting | Documented role | Boundary |
|---|---|---|
| England | NHS PDS integrated-products listing names HealthConnect 2020.1 at North West Anglia; NHS sources also document particular HealthShare GP Connect and NRL activity. | Dated product/customer or capability evidence; PDS approval does not establish current active use, ownership of Spine or approval for every national API. See England supplier evidence. |
| Scotland | Board-specific TrakCare and integration evidence, including Lothian's documented access to SCI Store eCasenotes through TrakCare. | A local application/interface role does not identify the supplier of every Scottish national service. See Scotland and InterSystems in Scotland. |
| Wales | InterSystems laboratory delivery and documented Welsh national-system interfaces. | Separate the deployed laboratory scope from WCP/WDS ownership and completion of the national rollout. See InterSystems in Wales. |
| Northern Ireland | Public procurement evidence for Caché licensing supporting the legacy NHAIS application. | This does not establish InterSystems as the supplier of NIDIS, encompass or NIPACS+. See InterSystems in Northern Ireland. |
Questions the Evidence Still Needs to Resolve
The principal open questions are the pathology pilot's later assurance/rollout dates, NIR adoption and IEP retirement by organisation, NIDIS phase 2 completion and the scope of remaining NHAIS functions, and site-level coverage of Scotland/Wales modernisation programmes. These are recorded in the Evidence Validation Queue, with confirmed facts retained in the country and diagnostics pages. The published evidence supports the map while these delivery questions remain open.
