UK Core Services Executive Summary

The four UK nations meet similar care needs through different digital services. England has a broad national interface catalogue; Scotland combines national services with health-board delivery; Wales shares selected England infrastructure; Northern Ireland combines HSC-wide services with separate GP systems. Connecting in one nation does not automatically connect a product in the others.

Start here for the quick comparison and access routes, then use the UK Core Services Matrix for the detail and the UK Healthcare Connectivity Landscape to understand how the services fit together. This summary follows every main topic in the matrix, grouping its 43 detailed functions into 38 comparison rows, with summary records explained in prose. The safeguarding repeat adds no new functions. Editorial alignment: 1 October 2026. The linked evidence retains its own review dates and rollout limits; this is not a fresh UK-wide deployment review.

How to Read Access

Each nation has a service description and a separate Access column to its right. The labels describe the named route:

  • System — a documented system-to-system API, message or automated feed (previously labelled S2S).
  • Portal — a person uses a website or application. A portal’s internal feeds do not establish an external clinical-data API.
  • Local — provider- or application-specific access; check the particular implementation.
  • Files — reference files or terminology packages, without an established live API here.
  • Other — the described telephone, administrative, framework or network route; no clinical software interface is asserted.
  • Planned — a proposed service or interface whose live availability is not established.
  • Unverified — this review does not establish the relevant interface; it does not prove none exists.

An Access cell can contain System and Portal where the description identifies both. Restricted, beta/pilot, deprecated and rollout qualifications still apply. System does not imply an open API, national coverage, full-record access, current Med IS support or InterSystems implementation.

Core Services

The foundations are patient identity, clinical exchange, prescriptions, diagnostics and record access. Organisation codes identify sites; they are different from system endpoint addresses.

✓ Current Med IS — light teal cells identify services reported as available with the current Med IS by the user on 21 September 2026. For pathology results, the mark applies to EDIFACT over MESH only; the FHIR reporting API remains a separate pilot. Unmarked cells have no current Med IS status recorded here. The same six service markers appear in the matrix; these highlights are separate from the Access labels.

Summary records—England’s SCR, Scotland’s ECS/KIS, Wales’ WGPR and NI’s NIECR GP-summary route—are not the full GP record. Requesting a test, receiving its report and accessing its images are separate functions. HSCN, SWAN, PSBA and HSC WAN provide network connectivity; they are not clinical interfaces.

Access, Registration and GP Transactions

Looking up a patient, signing in, changing the registered practice, reading a record and writing into it are separate actions. The detailed access comparison retains supplier and permission limits.

England’s catalogue is broader than its onboarding. GP Connect Update Record, Send Document and Appointment Management are closed to new developers; existing suppliers can onboard end users. Reading a record does not itself change GP registration. GP2GP is a separate transfer workflow, with cross-border limits.

Urgent Care Routing

Public access, assessment, finding a service, handing over care and arranging attendance are different steps. A delivered message does not prove clinical acceptance or an appointment. See the urgent-care detail.

New DoS Search clinical-service/triage operations are not yet live. England–Wales border referral and NHS 24 assurance learning are evidenced, but neither establishes a single UK urgent-care API.

Safety, Prevention and Care Planning

A plan, an adjustment need, a safeguarding flag, a vaccination event and a screening invitation are different records. Child protection and FGM remain separate rows. Safety detail holds the permissions and programme boundaries.

GIRFEC is Scotland’s wider wellbeing framework, not a software interface. England’s flag interfaces have separate access and migration states; a published successor does not prove that every older route has retired. Yellow Card’s common UK purpose does not establish current integration in every GP product.

Safeguarding

Child protection and FGM information flags are the safeguarding-specific functions already covered above, repeated here for quick reference. This is a focused view of existing entries, not a complete safeguarding-service inventory. Unverified national counterparts do not imply that safeguarding activity is absent. See the safeguarding detail.

Coding, Reporting and Payments

Shared codes, approved data extraction, achievement calculation and payment administration have different purposes. Reporting detail retains the national rules and transition dates.

The National Data Opt-Out concerns relevant care data in England, rather than residency alone, and does not govern direct-care sharing. An aggregate extract is not a general patient-record API; common fit-note purpose does not establish a shared reporting interface.

Using DSIC as the Comparison Lens

DSIC is an England requirements and assurance framework, not UK-wide certification. Use its GP-system responsibilities to ask whether a complete workflow is supported, then check the relevant nation’s service, supplier role, permissions and onboarding. A catalogue entry does not itself grant access or prove deployment. The detailed DSIC mapping separates directly mapped requirements from adjacent services.

What Actually Crosses Borders

The full reuse comparison distinguishes seven relationships:

  • Shared services: England and Wales use NHS login, Spine/PDS for Welsh prescribing and PCRM registration infrastructure. Permissions, configuration and contract scope remain distinct.
  • A platform supplied to another nation: NI’s BSO supplies FPPS into Wales. This is platform reuse, not proof of common payment rules or cross-border patient-data exchange.
  • Administrative exchange: Scotland’s NHSCR exchanges demographic/movement information with other UK registration authorities. That does not provide online access to the full clinical record.
  • Shared terminology: SNOMED CT UK Edition and dm+d support common meaning; national adoption and data completeness still vary.
  • Joint standards: FHIR UK Core is four-nation collaboration. A common specification does not establish a shared live API.
  • A UK service or common function: Yellow Card is UK-wide; fit-note processes share a purpose but retain jurisdiction and implementation differences.
  • Similar purpose only: login services, screening systems and GP-summary portals can be functional counterparts without compatible APIs.

The practical model is a common clinical workflow with nation-specific connections and assurance. Reuse must be demonstrated for the named service, role and operation.

Keeping This Summary and the Matrix Aligned

The matrix retains the detailed comparison. Both pages show the same six user-reported Current Med IS ticks, with e-RS and MESH sharing one summary cell. Review the summary, matrix and Landscape together when the content changes, including Access cells and the affected country detail. A repository check requires explicit review after changes to this summary or its 11 supporting pages. It also checks for missing comparison sections, inconsistent safeguarding repeats and mismatched Med IS service markers, cell placement or reported dates. It does not monitor external websites or automatically verify meaning.
