Gateway Health has implemented a new TrakCare community solution configured for Australian community health services; Gateway Health's annual report also records My Health Record integration with TrakCare, a TrakCare upgrade, and staff training. Gateway Health's CEO later described the project as the successful implementation of a new clinical information system with InterSystems. Current and customer-side evidence that TrakCare is used in an Australian regional community health service context. The launch page is vendor-published and the customer-side corroboration is a CEO LinkedIn post, not a formal Gateway Health publication. The annual report confirms TrakCare use but predates the named community-solution launch.

Servicio de Salud Metropolitano Sur and El Bosque APS
Official SSMS material describes TrakCare implementation in the El Bosque APS primary-care network and the SSMS reference/contrareference workflow using the TrakCare Interconsultas module, focused especially on primary-care doctors. Later SSMS network and GES referral material still references TrakCare in APS / referral-management context. Strong official evidence for TrakCare relevance to Chilean primary-care / APS and referral workflows in SSMS. Newer sources support continued workflow relevance but do not prove a named current El Bosque go-live state or a Chile-wide APS deployment. The SSMS network-design source also flags limited TrakCare profiles in APS and parallel systems.

Victoria community health
A historical InterSystems case study says the Victorian Department of Human Services selected TrakCare in community configuration for 22 community health agencies across Victoria. A later Victorian Healthcare Association paper states that the majority of registered community health services in Victoria use TrakCare as a client-management system. Historical and contemporary sector evidence that TrakCare supports Victorian community-health services. The newer source is a sector policy paper / inquiry exhibit rather than a Victorian Government system inventory. Use for community-health status, not site-by-site implementation scope.
Kuwait
DHAMAN primary healthcare centres and outpatient care
DHAMAN's TrakCare EMR agreement was reported as beginning with primary healthcare centres before extension to additional centres and hospitals, and a later press release says DHAMAN achieved HIMSS O-EMRAM Stage 6 using InterSystems TrakCare in an outpatient-care context. DHAMAN's current corporate site separately describes a network of 15 primary healthcare centers and 3 hospitals. Moderate evidence for TrakCare in a primary-healthcare-centre and outpatient-care network context, with primary-source support for DHAMAN's current PHC/hospital network scale. Current DHAMAN site-scope material does not name TrakCare. Use it to bound the network context only, paired with the TrakCare-specific trade and press-release sources.

Northern Territory public health
NT Health states Acacia uses TrakCare software purchased from InterSystems, and the NT Government newsroom describes Acacia as based on InterSystems TrakCare with continuity-of-care value for remote communities and remote clinics. Public-sector evidence for territory-wide TrakCare-based clinical record infrastructure spanning hospitals and remote/community settings. Official evidence is strong, but this is not a standalone PHC module claim; use as public-health and remote/community-care adjacent evidence.
Qatar
primary healthcare clinics
A 2009 academic paper describes pharmacists in four primary health care clinics in Qatar using TrakCare to document interventions. Current PHCC public sources found in this pass do not support a present TrakCare PHCC claim. Historical evidence of TrakCare use in Qatar primary-healthcare clinics. This is old academic evidence only. Current Qatar PHCC claims should not be made unless PHCC, MOPH, or procurement evidence names TrakCare.

Current Reading

The strongest public PHC/community-health evidence found in this pass is Gateway Health in Australia, SSMS / El Bosque APS in Chile, Victoria community health, and DHAMAN in Kuwait. Northern Territory Acacia is also important because it shows TrakCare-based public-health infrastructure reaching remote/community settings, but it is not a dedicated primary-care module claim.

General market-ranking signals have been removed from this evidence map because they do not meet the page boundary for PHC/community-health implementation evidence.

Qatar now has an explicit current-status boundary: the 2009 TrakCare paper is historical, and current PHCC public material found in this pass does not support carrying it forward as a live PHCC TrakCare claim.

Remaining Follow-up Evidence

  • Find a formal Gateway Health publication or annual-report reference for the 2026 community-solution go-live; the customer-side evidence found so far is the CEO's public LinkedIn corroboration.
  • Revalidate named current El Bosque APS live status and whether TrakCare scope extended across additional APS networks; newer SSMS material supports continued workflow relevance but not that precise claim.
  • Current Qatar PHCC claims need a current PHCC or Ministry of Public Health source naming TrakCare; current PHCC sources found in this pass do not support the claim.
  • For Victoria, find a Victorian Government operational system inventory or provider-level current source if site-by-site scope matters.
  • For DHAMAN, find a DHAMAN primary page or implementation record that names TrakCare if making a current live TrakCare scope claim.
